If your doctor did not provide you with a TRIAZ Instant Savings Card when he or she prescribed TRIAZ Foaming Cloths, we will send a card to you immediately. Call our customer service at 800-550-5115.

• Be sure to bring your card each time you go to the pharmacy to fill a TRIAZ Foaming Cloths prescription. Your card is good for savings on up to 3 prescription fills or until the program expires (whichever comes first).
• Present your savings card to the pharmacist and ask what your co-pay will be with the savings card. Have the pharmacist process your insurance to be sure.
• Be sure your doctor has prescribed TRIAZ Foaming Cloths. This card is not valid for any other products.

This card is valid for up to 3 TRIAZ Foaming Cloths prescriptions. Offer not valid for prescriptions reimbursed in whole or in part under Medicaid, Medicare, including Medicare Advantage and Part D prescription drug plans, or any other federal or state program (including state pharmaceutical assistance programs) or where prohibited, taxed, or otherwise restricted.
